| The Medicaid Infrastructure (MI) Development component of the grant will focus on health systems change to minimize health insurance and medical care related to barriers to employment, and will continue to research program development to support this systems change. |

Both the MI and CEO components will focus on barrier removal and include partnerships with key stakeholders, a communication and access plan, and a monitoring plan to assess the success of the program and state systems change over time.

The grant has four overall goals: |
- To develop consistent state-wide strategies and policies communicating a clear message of work as a valued outcome that will enhance an individual’s resources and quality of life, resulting in the seamless coordination of supports across state agencies on an individual level.

- To enhance choice and self-direction of services and supports by providing individuals with ready access to information that helps them identify and choose from available supports that will make it possible for them to work.

- To enhance the capacity of the service systems at the direct support level to provide or support the provision of a coordinated array of employment-related supports so that customers receive a consistent message that work pays and that work is the outcome of choice.

- Incorporation of access to health care services and information for individuals with disabilities and support providers at all levels of the service system to ensure integration between health-related and employment supports.
